What We Do
Cardea Health operates multiple programs targeting individuals who have experienced chronic homelessness and complex illness. We aim to develop a continuum of care that supports each client with the appropriate level of care and support they require to become safely and stably housed in the community.
Advocacy Work:
A pressing concern looms, threatening the viability of the current care model and undermining access to home-based services for all Medi-Cal beneficiaries. The HCBA waiver program has reached the state-level cap on slots. This has left thousands of individuals across the state languishing on a waiting list to receive lifesaving services. Institutionalizing individuals with intensive medical needs not only robs them of their dignity but also costs the state millions in unnecessary spending each year. DHCS estimates an annual savings of $67.5 million for current HCBA users who live in the community rather than institutions.
At Cardea, we are actively engaged in advocacy efforts with other community-based organizations to expand access for all Californians to receive care in the setting of their choice. The HCBA waiver is one of the only avenues for Californians to receive intense medical care in the comfort of their own home. Existing evidence and traction across the state strongly suggest that the HCBA waiver model works and should be expanded. Together, we can ensure that everyone has access to the care they need, where they need it most.
Our Model
Cardea Health adapts services to meet the needs of each individual, enabling our programs to shelter and care for people everywhere on the spectrum of social and clinical needs. Our clinical interventions prioritize the autonomy and dignity of our clients and respect each person’s right to make decisions about their life and care independently. Each clinical program leverages existing local, state and federal programs that exist to provide services to individuals who have experienced homelessness and complex illness or disability. Cardea Health works closely with community partners to support our patients in accessing the quality care they deserve, and provides the advocacy and persistence required to navigate our complex health care system.
Our Impact
Cardea’s care model has proven highly effective across various metrics, producing drastically improved health outcomes for residents and a better quality of life. It has led to an almost 80% reduction in emergency department visits, skilled nursing facility admissions, and inpatient admissions among individuals housed for 180 days or more. This success translates to an average cost savings of $175,000 per homeless individual with acute and chronic health needs who receives HCBA waiver services in a permanent supportive housing setting. Additionally, it has gained significant momentum statewide, serving as a blueprint for other organizations and counties to utilize HCBA waiver services similarly, ensuring the best possible care for this vulnerable population.
